Employee Satisfaction Survey

Use this template

This employee satisfaction survey shows in a few minutes how your staff feel about their work: overall job satisfaction, pay, benefits, management and career prospects. Responses arrive directly in Mailpro, ready to be read.

What is this employee satisfaction survey for?

Use it once or twice a year, or after a major change such as a reorganization, new leadership or an office move. Employees first describe their overall job satisfaction on a five-point scale, from very satisfied to very dissatisfied, then rate six topics: salary, overall benefits, health benefits, individual management, recognition and opportunities for advancement. Three yes/no questions then check whether they feel valued, whether they have the resources they need and whether colleagues' opinions are sought often enough; every "no" opens a field to explain why. An open comment box and a consent checkbox complete the form.

Who is it for?

  • HR teams running an annual job satisfaction survey
  • Owners of small businesses without an HR department who want structured feedback
  • Team leaders preparing annual reviews
  • Non-profits, schools and local authorities running a staff satisfaction survey

How to adapt it in Mailpro

  • Edit the questions and texts: add a topic such as remote work, working hours or training, or remove those that do not apply to your organization.
  • Use conditional questions so that the "please explain" field only appears after a "no".
  • For more candid answers, remove the name and title fields and switch on anonymous responses.
  • Add your logo and colors, then share the form via a link, a QR code in the break room or an internal Mailpro newsletter.

All responses arrive in Mailpro, where you can read them as they come in.

Tips

  • Announce the survey beforehand and explain how the results will be used: participation depends on it.
  • Keep the same questions from one year to the next so that results remain comparable.
  • Allow two weeks to respond and send a reminder halfway through.
  • Share a summary of the findings and the actions you have agreed on; that is what makes people answer next time.

Frequently asked questions

Which employee satisfaction survey questions matter most?

Start with overall satisfaction, then cover pay, benefits, management, recognition and growth, as this template does. A few yes/no questions with a follow-up field reveal the reasons behind a low score.

Can the survey be anonymous?

Yes. The template asks for name and department, but you can remove those fields and enable anonymous responses. Keep the department only if teams are large enough that no one can be identified.

Is this employee satisfaction survey template free?

Yes. You can use it free of charge, customize it in Mailpro and share it with your whole team.
