This employee feedback survey gives your team a structured way to share how they feel about their job, their manager and the company culture. It helps HR and management spot problems early and decide what to improve.

What is this employee feedback survey for?

Use it once or twice a year, after a reorganisation or whenever you sense a change in morale. Employees rate their overall job satisfaction, describe difficulties in their current position and suggest what culture, environment or policies they would change. Further questions cover pay and benefits and look closely at their direct manager: leadership, ownership and ability to motivate, with a "prefer not to answer" option on sensitive points. A final open field leaves room for anything else.

Who is it for?

  • HR teams running regular climate or engagement checks
  • Small business owners who want honest input from staff
  • Department heads preparing a team workshop
  • Associations and schools listening to their employees

How to adapt it in Mailpro

  • Edit the questions and texts, and add your logo and colors to make it recognisably yours.
  • Remove the identity fields and enable anonymous responses: people speak more freely when they cannot be identified.
  • If you keep the department question, use it to compare teams; conditional questions can ask for details only when someone reports an issue.
  • Share the link in an internal Mailpro newsletter, on the intranet or via a QR code in the break room. Answers arrive in Mailpro.

Tips

  • Explain clearly who will read the answers and what will happen next.
  • Avoid asking for department in very small teams if anonymity is promised; it would make people identifiable.
  • Run the same employee survey at regular intervals to measure progress.
  • Share results and an action plan, otherwise participation drops the next time.

Frequently asked questions

Should a staff feedback form be anonymous?

In most cases yes, especially for questions about managers and pay. Keep names only when the goal is an individual follow-up conversation.

Is this the same as an employee feedback form for reviews?

No. A review form evaluates one person's performance; this one collects employees' opinions about their workplace. You can adapt it either way.

Can I build my own employee survey template from it?

Yes. Add or remove sections, for example on training or remote work, and save it as your company's standard version.
