Use this templateThis online medical consent form lets patients provide their details, health history and consent before a procedure, from home and at their own pace. Your care team receives complete, readable information before the visit.
Send it a few days before a treatment or an operation. Over seven short pages, patients enter their identity, contact details and insurance information, then their blood type, vaccination status, allergies, current medication and known conditions. Follow-up questions appear only when relevant, for example to list allergies. A consent section presents statements on the procedure, anaesthesia, possible transfusion, follow-up instructions and fasting, each to be ticked individually, before the patient signs and dates the form.
Rules differ by country and by type of procedure. Check local requirements; some acts may still need a handwritten signature.
Yes. The statements already mention parents and guardians; add fields for the guardian's identity.
