Use this template

This yoga waiver form asks each participant to read and accept a set of statements about the physical nature of yoga, then confirm with their name, contact details, date and signature. It gives studios and instructors a tidy record of acceptance before the first class.

What is this yoga waiver form for?

Before joining a class, workshop or retreat, participants tick the statements they agree with: yoga involves physical effort and some risk of injury, they were advised to consult a physician, they can refuse hands-on adjustments at any time, they release the studio and its team from liability, disputes go through mediation and then arbitration, and they are fit to take part. This yoga liability waiver is completed online, so new students can fill it in at home rather than at the front desk.

Who is it for?

  • Yoga studios welcoming new members
  • Freelance teachers running classes in parks, gyms or rented rooms
  • Retreat and workshop organisers
  • Companies offering yoga sessions to their employees

How to adapt it in Mailpro

Adjust each statement to your own terms, replace "the institution" with your studio's name and add your logo and colours. You can add questions about injuries or pregnancy, shown only when relevant through conditional questions. Share the form with a link in your booking confirmation, a QR code at reception or in a Mailpro newsletter to your students; signed forms arrive in Mailpro.

Tips

  • Have the wording reviewed by a legal professional: this template does not replace legal advice, and liability rules vary by country.
  • Write the statements in plain language so participants understand what they accept.
  • Ask returning students to renew their yoga consent form each year.
  • Remind people that they should tell the teacher about any health condition before class.

Frequently asked questions

Is a yoga waiver legally binding?

It depends on local law and on how the document is written. It shows that the participant was informed, but it cannot exclude every form of liability; ask a lawyer to check it.

What is the difference between a yoga release form and a waiver?

The terms are often used interchangeably. Both record that the participant understands the risks and releases the organiser from certain claims.

Do children need a separate yoga waiver form?

Yes, for minors a parent or legal guardian should complete and sign it. Add a field for the guardian's name.
