Use this template

This association membership form lets new members join and pay their annual fee online in one step. They give their details, choose a membership category, say how they heard about you and whether they would like to volunteer; membership becomes active as soon as payment is made.

What is this membership application form for?

Paper forms, bank transfers to match and cash payments at events take up a lot of a volunteer treasurer's time. This association membership form simplifies the process. Four fee categories are offered: individual, family at the same address, student or apprentice, and supporting member. When someone chooses the family rate, an extra question asks for the first names and ages of family members. The form also asks where people heard about the association, useful for your communication, and identifies those who might help as volunteers. Before submitting, new members accept your statutes and can sign up for your newsletter.

Who is it for?

  • Sports clubs collecting annual subscriptions
  • Cultural, music and theatre associations
  • Parent, neighbourhood and residents' associations
  • Charities welcoming supporting members

How to adapt it in Mailpro

Enter your own fee amounts and rename the categories if needed; online payment is included so the fee is collected when the form is submitted. Keep the conditional question for family members, edit the texts and statutes paragraph, and add your logo and colours. Share the form by link on your website, by QR code at your stand or events, or in a Mailpro newsletter. New memberships arrive in Mailpro, and you can set tags on contacts according to their answers, for example to group future volunteers or families.

Tips

  • Use the same form every year for renewals so that your data remains consistent.
  • Explain briefly what the fee covers: insurance, activities, equipment.
  • Welcome new members with an email shortly after they join.
  • Check that the newsletter checkbox is not pre-ticked, so consent is freely given.

Frequently asked questions

Can I use this membership form template for an organization other than an association?

Yes. Clubs, cooperatives and networks can adapt the categories and texts to their own rules; it works as a membership form for any organisation.

Is payment mandatory?

The template is built for joining with immediate online payment. You can adapt the categories, for example to add a free one.

What about personal data?

Only collect what you need and follow the data protection rules that apply to you; the form does not replace that responsibility.
