Thank you for the time you spent with us. Your honest answers help us improve the workplace for those who stay. This questionnaire is confidential.
Use this templateThis exit interview questionnaire gathers honest feedback from employees who are leaving your organisation. Why they leave, what could have kept them and how they rated their experience: their answers help you improve the workplace for those who stay.
A departing employee often speaks more freely than a current one. The questionnaire asks for the department and length of service, then the main reason for leaving: a better offer, career development, pay, management, workload or personal reasons. An open question asks what could have made them stay, followed by a rating grid on several aspects of the job. The employee then says whether they would recommend you as an employer and whether they would consider coming back. Grouped together, these exit interview questions reveal trends you cannot see in one-to-one conversations.
Not necessarily. Many organisations use both: the questionnaire for comparable data, the conversation for nuance.
Responses can be anonymous, but in a very small team the department or length of service may still reveal identities. Remove those questions if needed.
The main reason for leaving and what could have made the person stay: they point directly to what you can change.
