The fastest way to learn the form builder is to build something real. Here we create an enquiry form for a charter company based in Falmouth Harbour, which takes bookings from clients in the United States, Canada and Europe as well as local corporate groups. You can adapt each step for a restaurant, a shop or a professional practice.

Step 1: decide what the form must do

The charter company wants enough information to send a quote, and nothing more. That means dates, group size, type of trip, contact details and any special requests. It also wants to ask, separately, whether the person would like to receive the newsletter.

Step 2: create the form

In Mailpro, open the form builder and start from a blank form or a template. Give it a clear internal name, such as “Charter enquiry 2026–27 season”.

Step 3: add the fields

  1. First name and last name (required).
  2. Email address (required).
  3. Mobile number with country code (optional). Add help text such as “Include your country code, for example +1 268 for Antigua and Barbuda.”
  4. Preferred dates (date pickers for start and end).
  5. Number of guests (a number field).
  6. Trip type: day sail, overnight to Barbuda, week-long charter, corporate event (a drop-down).
  7. Special requests (a text box).

Step 4: add branching logic

With conditional logic, show extra questions only when relevant. If someone picks “overnight to Barbuda”, ask whether they want to visit Codrington Lagoon or the pink-sand beach. If they choose “corporate event”, ask for the company name.

Under Antigua and Barbuda’s Data Protection Act, 2013, you should explain why you collect the data and get consent to use it. Add a short line such as “We use these details to prepare your quote.” Then add an unticked checkbox: “Send me news and seasonal offers by email.” Link to your privacy policy. Many of this company’s clients live in Canada or the EU, where CASL and the GDPR expect the same kind of explicit opt-in.

Step 6: connect the form to your lists

Use contact synchronisation so that respondents who ticked the box join the newsletter list, while everyone else is stored only as an enquiry. Set a notification so the booking team receives each enquiry by email.

Step 7: style and share

Add your logo and brand colours, then choose how to publish: a link in your Instagram profile, an embed on the website, or a QR code on the marina notice board. Mailpro’s sharing tools generate each of these.

Step 8: test and track

Fill in the form yourself on a phone and on a computer. Check that the notification arrives and the contact appears in the right list. After launch, form statistics show completion rates and where people drop off.

Keep the data you collect proportionate

Section 5 of the Data Protection Act, 2013 says personal data should be adequate but not excessive for its purpose. For an enquiry form, that rules out asking for dates of birth, passport numbers or payment details up front. Collect those later, through the right secure channel, only if the booking goes ahead.

Adapting the example

  • Restaurant: date, time, party size, dietary needs, occasion.
  • Retailer: loyalty sign-up with name, email, birthday month and preferred branch.
  • Professional firm: enquiry type and preferred contact time only; collect sensitive documents through a secure channel later.
