The quickest way to learn the form builder is to make something real. Here we build a booking-request form for a dive centre in Għar Lapsi, but the same steps apply to a restaurant reservation form, a seminar registration or a client intake form.

Step 1: Start a new form

In Mailpro, open the form builder and choose a blank form or a template. Give the form an internal name you will recognise later, such as “Dive booking 2026”.

Step 2: Add the fields you need, and no more

  • First name and last name
  • Email address
  • Mobile number, with a hint showing the international format (+356 followed by eight digits for Maltese numbers, or the diver’s own country code)
  • Preferred date (date picker)
  • Certification level (drop-down)
  • Number of divers (number field)
  • Language preference: English or Malti

GDPR’s data minimisation principle applies: don’t ask for passport numbers or dates of birth unless you truly need them at this stage.

Step 3: Add conditional logic

Show a “Do you need equipment hire?” question only to people who choose “Open Water” or below. Branching logic keeps the form short for experienced divers.

Add two separate, unticked checkboxes: “Send me dive news and offers by email” and “Send me offers by SMS”. Malta’s S.L. 586.01 requires written consent for both channels. Link to your privacy notice below them. The booking itself must work whether or not these boxes are ticked.

Step 5: Style it

Apply your logo, brand colours and a background photo of the site. Mailpro lets you customise form design without CSS.

Step 6: Set notifications and confirmations

Turn on an email alert so the dive centre is notified of each request, and send the diver an automatic confirmation email with next steps.

Step 7: Protect it

Enable CAPTCHA to block spam submissions, which busy tourism sites attract.

Before publishing: test on real devices

Fill in the form yourself on a phone and a laptop. Try a foreign phone number and a Maltese one, leave optional fields empty, and check that the confirmation email arrives, including in the spam folder of a Gmail or Outlook account.

Step 8: Publish

Embed the form on your website, add the link to your Instagram bio, or print a QR code on flyers left at partner hotels. See how to embed a form.

Step 9: Use the data

Responses flow into a Mailpro contact list. Divers who ticked the email box can receive your newsletter; everyone receives service messages about their booking. Data is held on Mailpro’s Swiss servers, outside US jurisdiction, which simplifies your GDPR transfer assessment.
